BSN, PHN preferred.Current license as a Registered Nurse in NJ.Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.Current CPR certification.Responsibilities:Performing comprehensive initial assessments of client status, including physical, psychosocial, and environmental parameters.Developing an individualized plan of care for each client with client input.Providing interventions with a focus on achieving realistic client outcomes.Conducting client reassessments and follow up visits.Maintaining compliant client records showing systematic assessments, planning interventions, and evaluations.Provide training, delegation, supervision and support to our staff of caregivers.Address any issues with respect to clients and or caregivers that may arise.Flexible work from home options available.Compensation: $40.00 - $50.00 per hour
